I just finally solved my issues with FIOS and my activation including not getting HBO or Cinemax, despite being subscribed to them. I'll outline the story and ultimate resolution below as I know there are several other posts on this topic in the forum.
New (and 1st) CableCard M-card from Verizon FIOS this week
Followed all HDHR setup steps
Did self-install from Verizon site
Input CC ID, Host ID, Data ID
Activation runs for a while, says success.
I have access to almost all the channels I subscribe to, except HBO and Cinemax.
Card status reads:
OOB Lock: success
Log file also indicates:
19700101-00:00:04 CableCARD: authentication status: authentication in progress
19700101-00:00:04 CableCARD: authentication status: authentication success, activation needed
So, spend an hour with Support last night, Matthew, who tired very hard to help trying all kinds of things, confirming all #s, etc. to no success. After more research last night on these forums and TIVO tried again this morning.
This morning I try to use VZ At Home Agent to redo the install - says it works, no joy.
I called in and this time w/ Monee (sp?), when I explained I was not getting HBO or Cinemax, which I understood had recently changed how VZ had to apply copy protection and that it appeared my card, while activated, was not correctly paired, she went right to work on performing the action I read about: a CC Reload or CableCard reload.
Ultimately, she had cancel the activation, 'reorder' the card (it actually appears as if an order for a CC was placed today in the system), reenter the CC ID (VZ also calls it the POD ID), Host ID and Data ID.
Initially this worked, but then dropped as seen from the log

I believe this failure occured because interestingly the Data ID changed from what I started with to a completely different number. Noting this I told her, she reentered the new ID and then she revalidated the card and, tada, working!
